The Following information needs to be completed for breeding records:

1. All mare information needs to be completed on Stallion contract
2. Signature.
3. Indicate how we should list the breeder on the AQHA Stallion Report. (i.e., your name or ranch name.)
4. Shipped Semen information form completed.

Please print and return the completed contract to Dana Hokana Quarter Horses for approval along with a copy of the mare's registration certificate (front & back). A copy of the contract will be returned to you for your files.
The shipped semen information form must be completed and returned with all fees to be paid prior to shipping.
